This application is intended to be completed by Family Caregivers who provide full-time, unpaid support to a family member, friend, or loved one who has a disability, chronic illness, or other "special health care need" as defined by the Lifespan Respite Act of 2006. 

The temporary respite care voucher you are applying for can be used only for respite care services, which is a temporary break for the primary Family Caregiver. A link to the Program Manual, which contains program terminology, guidelines, and requirements, can be found in the application.
